This explains what happens to the information you share through this website and its request forms. It is written plainly and kept short on purpose.

What this covers

This notice covers this website, the “request a call” form on it, and the inquiry form the practice runs on Facebook and Instagram. It does not cover the private records the office keeps about you as a patient, which the practice handles directly.

Please don’t send medical details here

These forms are a simple way to ask the office to call you back. They are not a secure place for health information. Please don’t include diagnoses, conditions, medications, or other sensitive medical details in them. If you need to talk through something health related, call the office at (248) 688-0917 and we’ll handle it there.

What we collect

On this website, the request form asks for your name, phone number, email address, and, if you choose, the best time to reach you. It also has a hidden field that helps block spam. On the Facebook and Instagram form, we collect your name, phone number, and email. Neither form asks for health information.

How it’s used

We use what you submit for one thing: so someone from Dr. Williams’ office can call you back about your interest in the practice. To do that, the information is shared with the office and with a service provider that helps the practice respond to new inquiries.

Services this site relies on

This website is hosted by Netlify, and the request form is processed by Netlify’s form service. If you use the form on Facebook or Instagram, Meta collects your responses and passes them to the office under Meta’s own terms. The contact page shows a map that loads from Google. This website does not use advertising or analytics trackers and does not set its own cookies.

If you would like to know what you submitted, correct it, or have it removed, call the office at (248) 688-0917 and ask.
